This research investigates the impact of deprivation on demographic inequalities in England and Wales among adults. Using demographic measures including the modal age at death, life expectancy, lifespan variation and mortality, it shows a negative correlation with deprivation as measured by the 2015 Index of Multiple Deprivation. Although it finds that life expectancy is increasing overall and the gap between men and women is lessening, improvements are slower paced in more deprived areas such that the gap between rich and poor is slowly worsening over time. Men are more adversely impacted by deprivation than women with the gap in period life expectancy at age 30 in 2015 between the top and bottom 1% of deprived neighbourhoods at 10.9 years for men and 8.4 years for women. Between 2001 and 2015 inequalities in male mortality rates at age 44 were 4.4 times greater in the most deprived 10% of neighbourhoods than those in the 10% least deprived and were much higher than in intervening deciles. The worst deprivation is concentrated in specific areas. For example, in 22 out of 326 English districts, 25% or more of neighbourhoods are in the most deprived 10% and in 5 districts it is 40% or above.

Summary
Most area-based initiatives (ABIs) – of which Sure Start Local Programmes (SSLPs) are a classic example – have needed to face the challenge of translating the concept of ‘deprived areas’ as targets for government intervention into actual boundaries ‘on the ground’. Chapter Two explained that, in the case of SSLPs, this responsibility was largely given to the local agencies who initiated the proposals for the establishment of SSLPs. They were required to identify local communities that contained a significant number of deprived families, had a degree of social coherence and contained a sufficient number of young children to make programmes viable (Glass, 1999). In practice this meant that the boundaries of many SSLPs did not correspond to any pre-existing area boundaries that would normally be used for collecting and disseminating indicators of economic, social, health and crime conditions, such as electoral wards or police beat areas for instance. This chapter describes the use that was made of Geographical Information Systems (GIS) to identify young children living in the SSLPs to take part in the Impact Study of the National Evaluation of Sure Start (NESS), and to adapt existing data or commission new data that would allow the nature of the SSLPs to be described by the Local Context Analysis (LCA) team, while also allowing assessment of their change over the lifetime of the evaluation.
The first stage in this process was to translate programme boundaries from paper maps into a digital form that could be used within GIS software. The method used for this and the challenges faced in its application are described. The second stage was to identify a key that could link households living within an SSLP to records that would identify the presence of young children and that could be used to link the SSLP's households to other types of data that could be used to derive profiles of the SSLPs. This key was the postcode system applied to all UK addresses. Postcodes could be used within a GIS to identify households within the programme area and also to assess the degree to which other area-specific units used for data collection and dissemination, such as Census wards or police authority crime beats, overlapped with the SSLPs.

Non-speaking people often rely on AAC (Augmentative and Alternative Communication) devices to assist them to communicate. These AAC devices are slow to operate, however, and as a result conversations can be very difficult and frequently break down. This is especially the case when the conversation partner is unfamiliar with this method of communication, and is a big obstacle to many people when they wish to conduct simple everyday transactions. A way of improving the performance of AAC devices by using scripts is discussed. A prototype system to test this idea was constructed, and a preliminary experiment performed with promising results. A practical AAC device which incorporates scripts was then developed, and is described.

The concept of formal or standardized tests for assessing function came to the fore in the 1960s. Katz et al. acknowledged the hierarchical nature of activities of daily living (ADL) such as eating, continence, transferring, going to the toilet, dressing and bathing in his ’Index of ADL’ and by 1968 ‘ADL’ was an accepted Index Medicus category. The definition of instrumental activities of daily living (IADL) began in 1969 with the work of Lawton and Brody who presented two scales to assess function which recognized the different degrees of complexity required for performing functional tasks. The first scale, taking life maintenance and activities essential for self-care as the primary level, was called the Physical Self-Maintenance Scale.

An 80-year-old widow with delusional infestation in association with post-herpetic neuralgia and EEG abnormalities in the left anterior parietal lobe responded to combined pimozide and carbamazepine. Aetiological factors are reviewed in relation to the literature.
